We aimed to investigate the impact of COVID-19 infection on maternal characteristics and obstetric and neonatal outcomes in a cohort of women in labor previously vaccinated who tested positive for SARS-CoV-2 infection, compared to aged-matched healthy controls. A retrospective case-control study was conducted among 66 women in labor. Clinical data were obtained from medical records. The attendance rates at childbirth and parenting classes, as well as the implementation of a birth plan, were significantly lower in the COVID-19 infection group (6.1% vs. 48.5%, <0.001; 6.1% vs. 33.3%, p = .005, respectively). Women with COVID-19 had a higher prevalence of prolonged postpartum hospital stay (33.3% vs. 9.1%, p = .016), and significantly higher prevalence of spontaneous preterm birth (27.3% vs. 1.09%, p = .006). Breastfeeding within the first 24 hr was also lower in women with COVID-19 (72.7% vs. 97.0%, p = .006). Maternal characteristics and neonatal outcomes are influenced by COVID-19 infection in vaccinated women. Complications include spontaneous preterm birth, prolonged postpartum hospital stay, and lack of breastfeeding within the first 24 hr. Childbirth education, parenting classes and implementing a birth plan may be associated with a decreased risk of COVID-19 infection.

A newborn's admission to the Neonatal Intensive Care Unit (NICU) can be both stress inducing and frightening for a parent or caregiver. With nursing being a constant calming presence, a trusting bond between the parents and nurses often becomes the lifeline to survive the NICU journey. This bond impacts not only the baby and family, but also promotes the institution's reputation within the community. In today's climate of healthcare professional shortages, the power of the nurses' connections to families cannot be overlooked. It is critical in all neonatal units, especially where parents are being approached to enroll their infant in clinical research.

RESUMO

Neonatal nurses play an essential role in small and sick newborn care. In the last few years, especially during the pandemic, neonatal mortality stayed relatively static. Recognition is growing that neonatal nurses represent a specialty that requires unique, consistent, competency-based training and education to provide the best possible care. The Council of International Neonatal Nurses, Inc collaborates with many global stakeholders to raise the standards of neonatal nursing care, especially in Africa.

AIM: To explore the experiences of partnership nursing among nurses when caring for children and young people with long-term conditions, and their families. BACKGROUND: Partnership nursing is promoted as a positive model of care among paediatric nurses, where shared roles and decision-making, parental participation, mutual trust and respect, communication and negotiation are valued to create positive care experiences and enhance patient outcomes. Little is known about how nurses use partnership with both the patient and the parents in this triad to deliver partnership nursing. DESIGN: A qualitative systematic review followed Joanna Briggs Institute meta-aggregation approach and has been reported according to PRISMA guidelines. METHODS: A comprehensive systematic search was conducted in seven electronic databases. Studies were assessed according to a pre-determined inclusion criteria. Qualitative findings with illustrative participant quotes were extracted from included studies and grouped into categories to inform overall synthesised findings. Methodological quality assessment was conducted. FINDINGS: A total of 5837 publications were screened, and 41 qualitative studies were included. Three overarching synthesised findings were identified: (1) Using education to promote feelings of safety and support, (2) Partnering to develop a strong therapeutic relationship and (3) Optimising communication underpinned by shared decision-making principles to deliver individualised care. CONCLUSION: Nurses demonstrated successful partnership in their practice, but focused on developing dyadic nurse-parent and dyadic nurse-child partnerships. Future practice development that creates a three-way triadic partnership may aid therapeutic relationships and shared decision-making. IMPLICATIONS FOR CLINICAL PRACTICE: Clinicians can reflect on how dyadic partnerships (focusing on the child or the parent) may exclude opportunities for coherent care. Further exploration in practice, policy and research as to how nurses determine child competency and child and parent level of engagement in triadic partnership may improve the potential of meaningful shared decision-making.

Introdução: O peso ao nascer tem grande influência sobre o desenvolvimento global da criança, que por sua vez sofre influência direta dos cuidados maternos. Objetivo: Identificar na literatura científica a percepção materna sobre o cuidado de recém-nascidos com baixo peso. Métodos: Trata-se de uma revisão sistemática de estudos qualitativos conduzida em conformidade com as diretrizes metodológicas do Instituto Joanna Briggs. A pesquisa foi realizada entre abril de 2021 e março de 2023 nas bases de dados PubMed, MedLine, BVS, Scielo, EMBASE e Web of Science, com os descritores: "percepção materna", "baixo peso ao nascer" e "cuidado da criança", em diferentes combinações. Incluíram-se estudos em português, inglês e espanhol, publicados entre 2013 e 2023, que tratassem do cuidado materno de recém-nascidos com baixo peso no contexto hospitalar ou domiciliar. Os resultados foram categorizados de acordo com a análise de conteúdo. Resultados: Foram identificados 1129 estudos, dos quais apenas 11 foram incluídos nesta revisão, nas seguintes categorias: sentimentos maternos, necessidades e desafios do cuidado, aleitamento materno, vínculo mãe-bebê e apoio social. Considerações finais: Mães de recém-nascidos com baixo peso experimentam insegurança e problemas gerais de adaptação que afetam os cuidados para suprimento das necessidades essenciais da criança. Apoio familiar e de profissionais de saúde podem reforçar a segurança materna e contribuir para o cuidado integral de recém-nascidos com baixo peso.

Métodos: Esta es una revisión sistemática de estudios cualitativos realizados de acuerdo con las pautas metodológicas del Instituto Joanna Briggs. La investigación fue realizada entre abril de 2021 y marzo de 2023 en las bases de datos PubMed, MedLine, BVS, Scielo, EMBASE y Web of Science, con los descriptores: "percepción materna", "bajo peso al nacer" y "cuidado del niño", en diferentes combinaciones. Se incluyeron estudios en portugués, inglés y español, publicados entre 2013 y 2023, que abordaran el cuidado materno de niños nacidos con bajo peso en el contexto hospitalario o domiciliario. Los resultados se clasificaron según el análisis de contenido. Resultados: Se identificaron 1129 estudios, de los cuales solo 11 fueron incluidos en esta revisión, en las siguientes categorías: sentimientos maternos, necesidades y desafíos de cuidado, lactancia materna, vínculo madre-hijo y apoyo social. Consideraciones finales: Las madres de niños nacidos con bajo peso experimentan inseguridad y problemas generales de adaptación que afectan los cuidados para satisfacer las necesidades esenciales del niño. El apoyo familiar y el apoyo de los profesionales de la salud pueden reforzar la seguridad materna y contribuir a la atención integral en salud de los niños nacidos con bajo peso.


Introduction: Birth weight has a great influence on the child's global development, which in turn is directly influenced by maternal care. Objective: To identify in the scientific literature the mother's perception about the care of children born with low birth weight. Methods: This is a systematic review of qualitative studies conducted in accordance with the methodological guidelines of the Joanna Briggs Institute. The research was carried out between April/2021 and March/2023 in the databases: PubMed, Medline, VHL, SciELO, EMBASE and Web of Science, with the descriptors: maternal perception, low birth weight and child care, in different combinations. Studies in Portuguese, English and Spanish, published between 2013 and 2023, that addressed the maternal care of children born with low birth weight in the hospital or home context were included. Results were categorized according to content analysis. Results: 1129 studies were identified, of which only 11 were included in this review, in the following categories: maternal feelings, care needs and challenges, breastfeeding, mother-infant bonding and social support. Final considerations: Mothers of children born with low birth weight experience insecurity and general adaptation problems that affect care to meet the child's essential needs. Family support and support from health professionals can reinforce maternal security and contribute to the comprehensive health care of low birth weight children.

ABSTRACT BACKGROUND AND OBJECTIVES: This study is justified by the interest of the area in question and the curiosity to know what methods and procedures are carried out on newborn pain, to help professionals and students to better understand the subject, in addition to showing the results obtained and help to improve care for this public. The objective of this study was to know the perceptions of nursing professionals working in the Neonatal Intensive Care Unit (NICU) and in the nursery about newborn (NB) pain and its management. METHODS: A descriptive, exploratory study with a qualitative approach. Technicians, assistants, and nurses who worked for at least three months in the NICU and nursery units of a philanthropic hospital participated in the research. Professionals who were on leave due to vacation or leave of any kind during the study period were excluded. A semi-structured interview was used, and a data analysis was carried out according to the content analysis proposed by Bardin. RESULTS: Twelve professionals, nine nursing technicians and three nurses participated in the research, of these, seven professionals working in the NICU and five in the nursery. From the analysis of the statements of the study participants, two categories emerged: perceptions of professionals regarding the identification and cause of pain in the NB; and NB pain assessment and management. CONCLUSION: The present study reveals the importance of identifying pain in the NB and its effective management, as the lack of appreciation for it can lead to more stress for the NB. Crying was the main characteristic identified by professionals when there is a NB with pain or some discomfort, therefore, mainly non-pharmacological methods are used for relief. Effective management should be seen as an indicator of the quality of care provided, ensuring humanized care, free from damage and with quality.


RESUMO JUSTIFICATIVA E OBJETIVOS: Este estudo se justifica pelo interesse da área em questão e pela curiosidade de saber quais os métodos e procedimentos realizados sobre a dor do recém-nascido, a fim de auxiliar profissionais e estudantes a entenderem melhor sobre o assunto, além de mostrar os resultados obtidos para ajudar na melhora do cuidado deste público. O objetivo deste estudo foi conhecer as percepções dos profissionais de enfermagem que atuam na Unidade de Terapia Intensiva Neonatal (UTIN) e no Berçário acerca da dor dos recém-nascidos (RN) e seu manejo. MÉTODOS: Estudo descritivo, exploratório, de abordagem qualitativa. Participaram da pesquisa técnicos, auxiliares e enfermeiros que atuavam, no mínimo, três meses nas UTIN e Berçário de um hospital filantrópico. Foram excluídos os profissionais que estavam afastados por motivo de férias ou licença de qualquer natureza durante o período do estudo. Utilizou-se a entrevista semiestruturada e a análise dos dados foi realizada de acordo com o proposto por Bardin. RESULTADOS: Participaram da pesquisa 12 profissionais, nove técnicas de enfermagem e três enfermeiras, destes, sete profissionais atuam na UTIN e cinco no berçário. A partir da análise dos depoimentos das participantes do estudo, emergiram duas categorias: percepções dos profissionais quanto à identificação e causa da dor do RN; e avaliação e manejo da dor. CONCLUSĆO: O presente estudo revelou a importância da identificação da dor no RN e o manejo eficaz, visto que a não valorização deste pode resultar em mais estresse para o RN. Notou-se que o choro é a principal característica identificada pelas profissionais quando se tem um RN com dor ou algum desconforto e que são utilizados principalmente os métodos não farmacológicos para o alívio. O manejo efetivo deve ser visto como um indicador de qualidade da assistência oferecida, garantindo um atendimento humanizado, livre de lesões e com qualidade.

Objetivo: analisar a percepção da equipe de enfermagem sobre o método canguru em uma maternidade de alto risco. Método: estudo exploratório, descritivo, de abordagem qualitativa, realizado por meio de entrevista semiestruturada com enfermeiros de uma maternidade de alto risco de Recife (PE), desenvolvida entre janeiro e fevereiro de 2020. As entrevistas foram transcritas e submetida a análise através da técnica de conteúdo de Bardin. Resultados: ocorreu a formulação de duas categorias, assistência de enfermagem no método canguru e benefícios e desafios encontrados no Método Canguru. As participantes relataram que os cuidados de enfermagem se baseiam principalmente em orientações as mães, a escassez de profissionais e baixa adesão foram evidenciados como principais desafios. Conclusão: constatou-se que a atuação de enfermagem no método canguru é um complexo processo, necessitando de uma equipe de enfermagem motivada e especializada


Objective: analyze the nursing team's perception of the kangaroo method in a high-risk maternity hospital. Method: exploratory, descriptive study, with a qualitative approach, carried out through semi-structured interviews with nurses from a high-risk maternity hospital in Recife (PE), carried out between January and February 2020. The interviews were transcribed and subjected to analysis using the content technique by Bardin. Results: two categories were formulated, nursing care in the kangaroo method and benefits and challenges found in the Kangaroo Method. Participants reported that nursing care is mainly based on guidance from mothers, the shortage of professionals and low adherence were highlighted as main challenges. Conclusion: it was found that nursing practice in the kangaroo method is a complex process, requiring a motivated and specialized nursing team


Objetivos: analizar la percepción del equipo de enfermería sobre el método canguro en una maternidad de alto riesgo. Método: Estudio exploratorio, descriptivo, con enfoque cualitativo, realizado a través de entrevistas semiestructuradas a enfermeros de una maternidad de alto riesgo de Recife (PE), realizado entre enero y febrero de 2020. Las entrevistas fueron transcritas y sometidas a análisis mediante el Técnica de contenidos de Bardin. Resultados: Se formularon dos categorías, los cuidados de enfermería en el método canguro y los beneficios y desafíos encontrados en el Método Canguro. Los participantes informaron que la atención de enfermería se basa principalmente en la orientación de las madres, la escasez de profesionales y la baja adherencia fueron destacados como principales desafíos. Conclusión: se encontró que la práctica de enfermería en el método canguro es un proceso complejo, que requiere de un equipo de enfermería motivado y especializado

Objetivo: construir e validar um formulário para a transição de cuidados com o neonato prematuro. Método: estudo metodológico realizado em três etapas: levantamento do referencial teórico-metodológico, análise semântica e validação de conteúdo, baseado no Modelo de Construção de Instrumentos e utilizando o método de Delphi, sendo considerado aprovado quando o índice de validação de conteúdo foi maior que 80%.Resultados: o formulário composto por seis domínios foi aprovado após três rodadas, com 64itens e alcançou uma aprovação média de 89%.Conclusão: o formulário de transição de cuidados foi validado quanto a face e conteúdo, disponibilizando uma nova tecnologia a ser utilizada para a transição de informações de forma padronizada e segura.


Objective:to construct and validate a form for transition of care for premature newborns. Method:a methodological study carried out in three steps: theoretical-methodological framework survey, semantic analysis and content validity, based on the instrument construction model and using the Delphi method, being considered approved when the Content Validity Index was greater than 80%. Results:the form consisting of six domains was approved after three rounds, with 64 items, and achieved a mean approval of 89%. Conclusion:the transition of care form was validated in terms of face and content, providing a new technology to be used for standardized and safe transition of information.

Método:estudio metodológico realizado en tres etapas: levantamiento del marco teórico-metodológico, análisis semántico y validación de contenido, con base en el modelo de construcción de instrumentos y mediante el método Delphi, considerándose aprobado cuando el índice de validación de contenido fue superior al 80%. Resultados:el formulario compuesto por seis dominios fue aprobado después de tres rondas, con 64 ítems, y alcanzó una tasa de aprobación promedio del 89%. Conclusión:el formulario de transición de atención fue validado en términos de apariencia y contenido, proporcionando una nueva tecnología para ser utilizada para la transición de información de forma estandarizada y segura.

Introdução: o nascimento de um bebê prematuro, por vezes, necessita de internação em unidade de terapia intensiva neonatal, o que resulta na mudança de toda dinâmica familiar. Isso ocorre devido ao longo período de hospitalização para o bebê e consequente permanência dos pais no ambiente hospitalar. Objetivo: conhecer os desafios da prematuridade e o papel do apoio social na percepção das mães durante o internamento do seu filho em uma unidade de terapia intensiva neonatal. Metodologia: estudo de abordagem qualitativa, cujos dados foram coletados mediante entrevistas, com sete mães, entre março e agosto de 2018. Os dados foram submetidos a análise de conteúdo proposta por Bardin. Resultados: verificou-se que entre os desafios mais comuns enfrentados pelas mães durante o período de internação estão relacionados ao medo da perda do filho hospitalizado, o apoio da equipe de enfermagem e da família as principais estratégias para enfrentar este momento. Conclusão: o reconhecimento dos desafios enfrentados e o apoio social por familiares e pela equipe de enfermagem são fundamentais a fim de minimizar as dificuldades enfrentadas pelas famílias dos recém-nascidos durante o processo de hospitalização.


Introduction: the birth of a premature baby sometimes requires hospitalization in a neonatal intensive care unit, which results in a change in all family dynamics. This occurs due to the long period of hospitalization for the baby and the consequent permanence of the parents in the hospital environment. Objective: to know the challenges of prematurity and the role of social support in the perception of mothers during their child's hospitalization in a neonatal intensive care unit. Methodology: study with a qualitative approach, whose data were collected through interviews with seven mothers, between March and August 2018. The data were submitted to content analysis proposed by Bardin. Results: it was found that among the most common challenges faced by mothers during the period of hospitalization are related to the fear of losing the hospitalized child, the support of the nursing team and the family are the main strategies to face this moment. Conclusion: recognition of the challenges faced and social support by family members and the nursing team are essential in order to minimize the difficulties faced by families of newborns during the hospitalization process.


Introducción: el nacimiento de un bebé prematuro en ocasiones requiere hospitalización en una unidad de cuidados intensivos neonatales, lo que resulta en cambios en toda la dinámica familiar. Esto ocurre debido al largo período de hospitalización del bebé y la consecuente estancia de los padres en el ambiente hospitalario. Objetivo: comprender los desafíos de la prematuridad y el papel del apoyo social en la percepción de las madres durante la hospitalización de su hijo en una unidad de cuidados intensivos neonatales. Metodología: estudio cualitativo, cuyos datos fueron recolectados a través de entrevistas a siete madres, entre marzo y agosto de 2018. Los datos fueron sometidos al análisis de contenido propuesto por Bardin. Resultados: se encontró que entre los desafíos más comunes que enfrentan las madres durante el período de hospitalización están relacionados con el miedo a perder a su hijo hospitalizado, siendo el apoyo del equipo de enfermería y de la familia las principales estrategias para afrontar este momento. Conclusión: el reconocimiento de los desafíos enfrentados y el apoyo social por parte de los familiares y del equipo de enfermería son fundamentales para minimizar las dificultades que enfrentan las familias de los recién nacidos durante el proceso de hospitalización.

Objetivo: analizar el patrón temporal y estimar las tasas de mortalidad en las primeras 24 horas de vida y por causas evitables en el estado de Pernambuco en el período de 2000 a 2021. Método: estudio ecológico, teniendo como unidad de análisis el trimestre. La fuente de datos se constituyó por el Sistema de Informaciones sobre Mortalidad y el Sistema de Informaciones sobre Nacidos Vivos. El modelado de series temporales se realizó según el Modelo Autorregresivo Integrado de Promedio Móvil. Resultados: se registraron 14.462 óbitos en las primeras 24 horas de vida, siendo 11.110 (el 76,8%) evitables. Se observa para los pronósticos ( forecasts) que la tasa de mortalidad en las primeras 24 horas de vida registro una variación de 3,3 a 2,4 por 1.000 nacidos vivos, y la tasa de mortalidad por causas evitables de 2,3 a 1,8 por 1.000 nacidos vivos. Conclusión: la predicción sugirió avances en la reducción de la mortalidad en las primeras 24 horas de vida en el estado y por causas evitables. Los modelos ARIMA presentaron estimaciones satisfactorias para las tasas de mortalidad y por causas evitables en las primeras 24 horas de vida.

The time series modeling was conducted according to the Autoregressive Integrated Moving Average Model. Results: 14,462 deaths were recorded in the first 24 hours of life, 11,110 (76.8%) of which being preventable. It is observed from the forecasts that the mortality rate in the first 24 hours of life ranged from 3.3 to 2.4 per 1,000 live births, and the mortality rate from preventable causes ranged from 2.3 to 1.8 per 1,000 live births. Conclusion: the prediction suggested progress in reducing mortality in the first 24 hours of life in the state and from preventable causes. The ARIMA models presented satisfactory estimates for mortality rates and preventable causes in the first 24 hours of life.

Resumo Enquadramento: Promover o contacto físico, especialmente o pele a pele, de forma imediata ou precoce em recém-nascidos prematuros é ainda um desafio. Objetivo: Verificar a prevalência de contacto físico entre mãe e recém-nascido prematuro no nascimento e associar o tipo de contacto físico realizado pelo binómio na sala de parto com a prevalência do aleitamento materno exclusivo na alta hospitalar. Metodologia: Estudo transversal com amostra não probabilística de 78 mães e 79 prematuros. A colheita de dados foi efetuada de julho a novembro de 2017, por meio de entrevista e consulta em prontuário e os dados foram analisados conforme a estatística descritiva e Regressão de Poisson com variância robusta. Resultados: A prevalência de qualquer tipo de contacto físico realizado na sala de parto foi de 51,9%, sem diferença estatística entre o tipo de contacto físico realizado entre mãe e bebé com o aleitamento materno exclusivo na alta. Conclusão: O contacto físico pelo toque e beijo no momento do parto não influenciou na prevalência de aleitamento materno exclusivo na alta hospitalar.


Abstract Background: Promoting physical contact, especially skin-to-skin contact, immediately or as soon as possible in preterm newborns is still a challenge. Objective: To determine the prevalence of physical contact between mothers and preterm newborns at birth and to analyze the association between the type of physical contact between them in the delivery room and the prevalence of exclusive breastfeeding at hospital discharge. Methodology: Cross-sectional study with a nonprobability sample of 78 mothers and 79 preterm newborns. Data were collected from July to November 2017, through interviews and medical records, and analyzed through descriptive statistics and Poisson Regression with robust variance. Results: The prevalence of any type of physical contact in the delivery room was 51.9%, without statistical difference between the type of physical contact between mothers and infants and exclusive breastfeeding at discharge. Conclusion: Physical contact by touching and kissing after birth did not influence the prevalence of exclusive breastfeeding at hospital discharge.


Resumen Marco contextual: Promover el contacto físico, especialmente piel con piel, de forma inmediata o precoz en recién nacidos prematuros sigue siendo un desafío. Objetivo: Comprobar la prevalencia del contacto físico entre la madre y el recién nacido prematuro en el momento del nacimiento y asociar el tipo de contacto físico realizado por el binomio en la sala de partos con la prevalencia de la lactancia materna exclusiva en el momento del alta hospitalaria. Metodología: Estudio transversal con una muestra no probabilística de 78 madres y 79 recién nacidos prematuros. La recogida de datos se realizó de julio a noviembre de 2017, mediante entrevista y consulta en antecedentes clínicos, y los datos se analizaron según la estadística descriptiva y la regresión de Poisson con varianza robusta. Resultados: La prevalencia de cualquier tipo de contacto físico realizado en la sala de partos fue del 51,9%, sin diferencias estadísticas entre el tipo de contacto físico realizado entre la madre y el bebé, y la lactancia materna exclusiva en el momento del alta. Conclusión: El contacto físico mediante caricias y besos durante el parto no influyó en la prevalencia de la lactancia materna exclusiva en el momento del alta hospitalaria.

Objetivo: describir el proceso de diseño e implementación de un protocolo de atención para la primera hora de vida del recién nacido prematuro. Método: investigación participativa, que utilizó el marco de la ciencia de la implementación y los dominios del Consolidated Framework for Implementation Research. Estudio realizado en un hospital escuela del sureste de Brasil, con la participación del equipo multidisciplinario y de los gestores. El estudio se organizó en seis etapas, mediante del ciclo de mejora continua (Plan, Do, Check, Act): diagnóstico situacional; elaboración del protocolo; capacitaciones; implementación del protocolo; relevamiento de barreras y facilitadores; seguimiento y revisión del protocolo. Los datos fueron analizados mediante estadística descriptiva y análisis de contenido. Resultados: el primer protocolo de la Hora Dorada de la institución fue organizado por el equipo multidisciplinario a partir de un enfoque colectivo y dialógico. El protocolo priorizó la estabilidad cardiorrespiratoria, la prevención de hipotermia, hipoglucemia e infección. Después de cuatro meses de capacitación e implementación, el protocolo fue evaluado como una intervención de calidad, necesaria para el servicio, de bajo costo y de poca complejidad. La principal sugerencia de mejora fue realizar actividades educativas frecuentes. Conclusión: la implementación generó cambios e inició un proceso de mejora de la calidad de la atención neonatal, es necesario que la capacitación sea continua para lograr mayor adherencia y mejores resultados.


Objective: describe the process of designing and implementing a care protocol for the first hour of life of premature newborns. Method: a participatory research study using an implementation science framework, the Consolidated Framework for Implementation Research (CFIR) was employed to determine drivers and facilitators of implementation success of the Golden Hour protocol for newborns at a large university hospital in southeastern Brazil. A multi-professional team, including first line providers and managers participated in six stages of quality improvement: situational diagnosis; protocol elaboration; training protocol implementation; barrier and facilitator assessment; and protocol monitoring and review. Qualitative and monitoring data collected across these six stages were analyzed using descriptive statistics and content analysis. Results: the institution's Golden Hour protocol was organized by the multi-professional team based on a collective and dialogical approach. The protocol prioritized the infant's cardiopulmonary stability, as well as prevention of hypothermia, hypoglycemia and infection. After four months of implementation, the care team was evaluated the protocol as a good quality intervention, necessary for the service, low-cost and not very complex. One suggested improvement recommended was to carry out refresher training to address staff turnover. Conclusion: implementation of the Golden Hour protocol introduced an appropriate and feasible neonatal care quality improvement process, which requires periodic refresher training to ensure greater adherence and better neonatal results.

Estudo realizado em hospital universitário no sudeste do Brasil, com participação da equipe multiprofissional e gestores. O estudo foi organizado em seis etapas, por meio do ciclo de melhoria contínua (Plan, Do, Check, Act): diagnóstico situacional; elaboração do protocolo; treinamentos; implementação do protocolo; levantamento de barreiras e facilitadores; monitoramento e revisão do protocolo. Os dados foram analisados por estatística descritiva e análise de conteúdo. Resultados: o primeiro protocolo Hora Ouro da instituição foi organizado pela equipe multiprofissional a partir de uma abordagem coletiva e dialógica. O protocolo priorizou a estabilidade cardiorrespiratória, prevenção de hipotermia, de hipoglicemia e de infecção. Após treinamento e implementação por quatro meses, o protocolo foi avaliado como uma intervenção de qualidade, necessária ao serviço, de baixo custo e pouco complexa. A principal sugestão de melhoria foi realizar ações educativas frequentes. Conclusão: a implementação provocou mudanças e iniciou um processo de melhoria da qualidade da assistência neonatal, sendo necessária a manutenção dos treinamentos para maior adesão e melhores resultados.

BACKGROUND: Low birth weight (LBW) is associated with short and long-term consequences including neonatal mortality and disability. Effective linkages in the continuum of care (CoC) for newborns at the health facility, community (primary care) and home care levels have a high tendency of minimizing adverse events associated with LBW. But it is unclear how these linkages work and what factors influence the CoC process in Ghana as literature is scarce on the views of health professionals and families of LBW infants regarding the CoC. Therefore, this study elicited the drivers influencing the CoC for LBW infants in Ghana and how linkages in the CoC could be strengthened to optimize quality of care. METHODS: A constructivist grounded theory study design was used. Data was collected between September 2020 to February 2021. A total of 25 interviews were conducted with 11 family members of LBW infants born in a secondary referral hospital in Ghana, 9 healthcare professionals and 7 healthcare managers. Audio recordings were transcribed verbatim, analyzed using initial and focused coding. Constant comparative techniques, theoretical memos, and diagramming were employed until theoretical saturation was determined. RESULTS: Emerging from the analysis was a theoretical model describing ten major themes along the care continuum for LBW infants, broadly categorized into health systems and family-systems drivers. In this paper, we focused on the former. Discharge, review, and referral systems were neither well-structured nor properly coordinated. Efficient dissemination and implementation of guidelines and supportive supervision contributed to higher staff motivation while insufficient investments and coordination of care activities limited training opportunities and human resource. A smooth transition between care levels is hampered by procedural, administrative, logistics, infrastructural and socio-economic barriers. CONCLUSION: A coordinated care process established on effective communication across different care levels, referral planning, staff supervision, decreased staff shuffling, routine in-service training, staff motivation and institutional commitment are necessary to achieve an effective care continuum for LBW infants and their families.

AIM: To assess the inter-rater reliability of modified Downes' scores assigned by physicians and nurses in the Ethiopian Neonatal Network and to calculate the concordance of score-based treatment for preterm infants with respiratory distress. METHODS: We included preterm infants admitted from June 2020 to July 2021 to four tertiary neonatal intensive care units (NICUs) of the Ethiopian Neonatal Network that presented with respiratory distress. We calculated the kappa statistic to determine the nurse and physician correlation for each component of the modified Downes' score and total score on admission and evaluated the concordance of scores above and below the treatment threshold of 4. RESULTS: Of the 1151 eligible infants admitted, 817 infants (71%) had scores reported concurrently and independently by nurse and physician. The kappa statistic for modified Downes' score components ranged from 0.88 to 0.92 and was 0.89 for the total score. There was 98% concordance for score-based treatment. CONCLUSION: Incorporation of the modified Downes' score on admission for preterm infants with respiratory distress was feasible in tertiary NICUs in Ethiopia. The kappa statistics showed near-perfect agreement between nurse and physician assessments, translating to a very high degree of concordance in score-based treatment recommendations. These results highlight an opportunity for task-shifting assessments and empowering nurses.

Background: The measurement of the coping difficulties of parents of premature infants after discharge provides objective data for nurses to prepare infants for discharge. However, no Chinese scale has been developed to measure parents' coping difficulties after their premature infants are discharged. Aim: To translate the parent version of the Post-Discharge Coping Difficulty Scale (Ped-PDCDS) from English to Chinese and test the reliability and validity of the Chinese version in parents of premature infants. Methods: A multicenter cross-sectional study of 356 parents of premature infants was conducted. The scale was symmetrically translated. Validity was evaluated in terms of content, construct, discriminant, and convergent validities. Reliability was assessed in terms of internal consistency, split-half reliability, and test-retest reliability. Results: The Chinese Ped-PDCDS finally contained 11 items. Exploratory and confirmatory factor analyses results showed that the Chinese Ped-PDCDS had three dimensions, and the convergent and discriminant validities of the scale was satisfactory. The overall reliability, split-half reliability, and test-retest reliability of the scale was 0.85, 0.92, and 0.84, respectively. Conclusion: The Chinese Ped-PDCDS has adequate psychometric properties, and is an easy and appropriate instrument for measuring parents' difficulty in coping with premature infants.

AIM: To describe parents' past and present experiences of their newborn infant's therapeutic hypothermia (TH) treatment after perinatal asphyxia 10-13 years after the event. BACKGROUND: Newborn infants are treated with TH following perinatal asphyxia to improve neurodevelopmental outcomes. DESIGN: A qualitative descriptive design using focus groups (FGs). METHODS: Twenty one parents to 15 newborn infants treated with TH between 2007 and 2009 participated in five FGs. The FGs were transcribed verbatim and analysed using framework approach. The SRQR checklist was followed for study reporting. RESULTS: Two main categories were identified: hardships and reliefs during TH treatment and struggles of everyday life. Both categories include three subcategories, the first: (1) concern and gratitude for the unrecognized treatment, (2) insufficiency of information and proposed participation and (3) NICU nurses instilled security and hope. The second with subcategories: (1) unprocessed experiences of the TH treatment, (2) later challenges at school and (3) existential and psychological challenges in everyday life. CONCLUSION: TH of their newborns affected the parents psychologically not only during the treatment, but lasted months and years later. Information and communication with health care professionals and school management were inefficient and inadequate. The parents' concerns could be prevented by an improved identification and understanding of the problems and the needs of the infants and their families before discharge. RELEVANCE FOR CLINICAL PRACTICE: Through more personalized and efficient preparation and communication by the nursing staff before discharge, many of the parents' worries and problems could be reduced. Check-up of parents' needs of psychosocial support before and after discharge and offering counselling should become routine. Also, nurses at Well-Baby Clinics and in school health care should receive knowledge about TH treatment and the challenges the children and the parents experience. PATIENT OR PUBLIC CONTRIBUTION: Participation of parents was limited to the data provided through interviews.

Background: Orogastric (OG) and nasogastric (NG) tubes are frequently used in the NICU. Obtaining a relatively accurate estimated length before insertion could significantly reduce complications. While previous studies have mainly focused on the NG tube, OG tubes are more commonly used in China. Purpose: The objective was to determine whether there were differences in the rate of accurate placement among the adapted nose-ear-xiphoid (NEX) method, nose-ear-midway to the umbilicus (NEMU) method, and weight-based (WB) equation in estimating the OG tube insertion distance. Methods: A randomized, controlled, open-label clinical trial to compare the three methods was conducted in a single center. After enrollment, newborns were randomly assigned into three groups. By radiological assessment, the anatomical region for OG tube placement was analyzed. The primary metric was the tip within the gastric body, and the second metric was strictly accurate placement defined as the tube was not looped back within the stomach and the end was located more than 2 cm but less than 5 cm into the stomach, referred to as T10. Results: This study recruited 156 newborns with the majority being preterm infants (n = 96; 61.5 percent), with an average birth weight of 2,200.8 ± 757.8 g. For the WB equation, 96.2 percent (50 cases) of the OG tubes were placed within the stomach, and the rates were 78.8 percent (41 cases) in the adapted NEX and NEMU methods. The strictly accurate placement rates were highest for the WB equation at 80.8 percent (42/52), followed by the adapted NEX method at 65.4 percent (34/52), and the NEMU method at 57.7 percent (30/52). Conclusion: The WB equation for estimating the insertion depth of the OG tube in newborn infants resulted in more precise placement compared to the adapted NEX and NEMU methods.

BACKGROUND: Neonatal pain assessment (NPA) represents a huge global problem of essential importance, as a timely and accurate assessment of neonatal pain is indispensable for implementing pain management. PURPOSE: To investigate the consistency of pain scores derived through video-based NPA (VB-NPA) and on-site NPA (OS-NPA), providing the scientific foundation and feasibility of adopting VB-NPA results in a real-world scenario as the gold standard for neonatal pain in clinical studies and labels for artificial intelligence (AI)-based NPA (AI-NPA) applications. SETTING: A total of 598 neonates were recruited from a pediatric hospital in China. METHODS: This observational study recorded 598 neonates who underwent one of 10 painful procedures, including arterial blood sampling, heel blood sampling, fingertip blood sampling, intravenous injection, subcutaneous injection, peripheral intravenous cannulation, nasopharyngeal suctioning, retention enema, adhesive removal, and wound dressing. Two experienced nurses performed OS-NPA and VB-NPA at a 10-day interval through double-blind scoring using the Neonatal Infant Pain Scale to evaluate the pain level of the neonates. Intra-rater and inter-rater reliability were calculated and analyzed, and a paired samples t-test was used to explore the bias and consistency of the assessors' pain scores derived through OS-NPA and VB-NPA. The impact of different label sources was evaluated using three state-of-the-art AI methods trained with labels given by OS-NPA and VB-NPA, respectively. RESULTS: The intra-rater reliability of the same assessor was 0.976-0.983 across different times, as measured by the intraclass correlation coefficient. The inter-rater reliability was 0.983 for single measures and 0.992 for average measures. No significant differences were observed between the OS-NPA scores and the assessment of an independent VB-NPA assessor. The different label sources only caused a limited accuracy loss of 0.022-0.044 for the three AI methods. CONCLUSION: VB-NPA in a real-world scenario is an effective way to assess neonatal pain due to its high intra-rater and inter-rater reliability compared to OS-NPA and could be used for the labeling of large-scale NPA video databases for clinical studies and AI training.
